What Treatment in India Really Costs — and Why We Don’t Publish a Single Procedure Price

Search for the cost of almost any operation in India and you will find pages quoting numbers down to the last dollar, sometimes literally, like a knee replacement at “$1,834”. The precision is the tell. Nobody knows the cost of your operation to four significant figures before a specialist has read your file, and the sites publishing those numbers know it, which is why the fine print says “for reference purpose only.”

This page explains how treatment costs in India are put together, how to read a real quote, and which numbers can honestly be published in advance. It is the framework we use ourselves when we obtain itemized quotes for patients.

What actually drives the number

Two patients having “the same” operation can receive honestly different quotes, because six inputs move the total:

1. The hospital category and city. A large corporate hospital in a metro, a specialist private hospital in a tier-2 city, and a trust hospital can quote meaningfully different rates for the same procedure with comparable clinical standards. Lower land, staffing and overhead costs in tier-2 cities feed straight into package prices.
2. The surgeon and team. Senior surgeons with deep case volumes command higher fees. This is one of the few line items where paying more buys something you can verify: ask about case volume for your specific procedure.
3. The implant or device class. For joint replacements, cardiac procedures and spine surgery, the implant is often the single largest line item, and the same hospital will quote several implant classes at different totals. A quote that doesn’t name the implant class is incomplete.
4. Room category and length of stay. Packages include a set number of nights in a set room category. General ward, twin sharing, private and deluxe rooms carry different rates, and in most Indian hospitals the room category also scales other charges.
5. Your own health. Diabetes, heart conditions, obesity and other factors can extend stays, add specialist consultations, and move a case from a standard package to an individually priced plan. A real quote therefore comes after a specialist has read your reports, not before.
6. What’s excluded. The headline number is only half the quote. The other half is the exclusion list: implant upgrades, extra days, ICU beyond an allowance, blood products, complication management, post-discharge medicines. Honest quotes state exclusions plainly; padded ones bury them.

How to read a quote

A quote you can rely on and compare has these properties:

• Itemized, with surgeon, anesthesia, theatre, implant class, room nights and tests on separate lines
• On hospital letterhead, with the hospital’s own contact details, so you can verify it directly with the international billing desk
• For your case, issued after a specialist has read your reports, not a brochure rate
• Explicit about exclusions, in writing
• Stable in writing, a rate the hospital will lock for your admission window

If any of those is missing, you don’t yet have a price; you have marketing.

For ballpark orientation before that point, ranges are legitimate. We publish wide, dated, methodology-stated ranges for common procedures like knee replacement, hip replacement, heart bypass and angioplasty. Ranges tell you whether a quote is plausible. They are not prices.

The number nobody publishes: what the middleman costs

Most medical travel services are “free” to the patient because the hospital pays them a commission, and that commission travels inside the treatment bill you pay. Industry reporting describes facilitator commissions running from roughly 7.5% to 30% of the treatment price, with quotes from legacy brokers carrying 25–35% padding. You never see the line item, because nobody itemizes it.

That is why “free help” and “instant quotes” dominate this industry’s marketing: the price of the help sits where you cannot compare it.

The numbers we can publish honestly

• $150 for a written independent second opinion
• $1,000 flat for full journey coordination, milestone-gated

We obtain the hospital costs for your case: itemized, in writing, from more than one hospital, with the exclusions stated, so the comparison is yours to make.
